I, hereby give my consent for the registration of our school in the Zenith Bank Delta Principals Cup. I understand and agree to comply with all the rules, regulations, and guidelines set forth by the organizing committee for the duration of the tournament.

I acknowledge that the tournament organizers, sponsors, and partners hold the right to modify the tournament schedule, format, or rules if deemed necessary, and I agree to abide by such changes.

I confirm that our school’s participation in the Zenith Bank Delta Principals Cup will be in adherence to the eligibility criteria outlined by the organizing committee. I affirm that our players are bona fide students of our school and meet the age and academic requirements for participation.

I understand that our school will be responsible for covering any costs associated with our team’s participation, including but not limited to transportation, accommodation, uniforms, and equipment.

I grant the tournament organizers and their media partners the right to capture photographs, videos, and other media of our team and its members during the tournament for promotional and marketing purposes.

I agree to ensure the safety and well-being of our players throughout the tournament, and I release the tournament organizers, sponsors, partners, and the venue from any liability arising from injuries or incidents that may occur during the tournament.

I acknowledge that any breach of the rules or unsportsmanlike conduct by our team or its members may result in disciplinary actions, including disqualification from the tournament.

I have read and understood the terms and conditions outlined in this consent form, and I agree to abide by them for the duration of the Zenith Bank Delta Principals Cup.
